Community Health Assessment

Contact us

Public health is about preventing injuries and illness, promoting wellness and encouraging healthy behavior.

The Community Health Assessment plays a key role. The CHA tells the story of health in Okaloosa County. It shares the wonderful things about our community and the ways we can improve.

The CHA is produced every five years. The CHA is used by the community to identify health priorities. Community Health Improvement Plan (CHIP) groups then form and work together to set goals and take steps to create a healthier Okaloosa.

You can help make Okaloosa County a healthier place to live, learn, work and play!
