May 07, 2021

FORT WALTON BEACH, Fla.— Florida Department of Health in Okaloosa County (DOH-Okaloosa) is pleased to announce a COVID-19 vaccination program for homebound individuals in Okaloosa County. DOH-Okaloosa will visit the homes of homebound individuals to administer the vaccine.

Persons who are eligible for this program are Okaloosa County residents:

  • Who have not been vaccinated with a COVID-19 vaccine; and

To sign-up for the program, email DOHOkaloosa@flhealth.gov or call 850-344-0566. Please include a name and a contact phone number when emailing or calling.

A nurse from the health department will then contact the person at the phone number provided to verify the persons homebound condition and discuss if other members of the household or caregivers need vaccination. The nurse will also screen all individuals requesting the vaccine for any contraindications.

Following the initial assessment, the nurse will schedule the homebound vaccination visit. The type of vaccine being administered will be determined in the initial assessment based on screening criteria.
