The Community Health Assessment (CHA) (PDF 1MB) plays a key role. The CHA tells the story of health in Okaloosa County. It shares the wonderful things about our community and the ways we can improve.

The CHA is produced every three-five years. The CHA is used by the community to identify health priorities. Community Health Improvement Plan (CHIP) groups then form and work together to set goals and take steps to create a healthier Okaloosa.
