COVID-19 Vaccine 

DOH-Okaloosa is offering COVID-19 vaccinations in Fort Walton Beach (Monday, Tuesday, Wednesday) and Crestview (Thursday only).  Appointments are required.

Moderna, Pfizer, Novavax and Johnson & Johnson (Janssen) are available for a Primary Series for 18 years and older.  Pfizer is available for 5 years and older.  

COVID Bivalent (BA.4/BA.5) Pfizer and Moderna boosters are available for those that have completed their primary series or received a previous booster at least 2 months ago and are 18 years and older. Information about vaccine boosters

1. I lost my vaccine card

If you lost or misplaced your vaccine card, you can stop by the health department's Immunization's Office to receive your record from Florida SHOTS.
