Transferring to or from this Area

If you are moving from another state to Okaloosa County, you can transfer your immunization information to FL Shots by presenting your current immunization record from your previous state.

The record will be transferred into FL Shots and you will be issued a new Florida immunization record. No appointments are necessary, but please allow three business days for your form to be completed. If you are transferring a child into the Okaloosa County school district and need information on school physicals and a list of providers, please see School Health.

If you need a copy of immunization records from DOH-Okaloosa, no appointment is required; however, depending on clinic volume we may not be able to provide the documents immediately and the parent may need to return to pick it up.  Generally, requests are completed on the spot, the same day.

For immunization records/DH 680, parents may walk in and complete a request form or download FL 680 Request Form (pdf 67 kb) and fax to 850-833-3442.
