If you are moving from another state to Okaloosa County, we can transfer your immunization information to FL Shots by using your current immunization record from any state.

This record will be transferred into FL Shots and a new certified Florida Immunization DH 680 record will be issued. No appointments are necessary however depending on clinic volume we may not be able to provide the documents immediately and the parent may need to return to pick it up.

For DH 680 requests, parents may walk-in and complete a request form or download FL 680 Request Form  (PDF160KB) and bring to the health department during immunization operating hours. Effective January 3, 2024 there is a $15 charge for records transcription and issuing the certified DH 680 form for each request.

Immunization records request using signed release of medical information may be faxed to 850-833-3442.

Immunizations are provided free for children, ages birth through 18 years, through the Vaccines for Children Program (VFC). Before a child may enter childcare or attend a school, proof of vaccines must be provided. Immunizations can protect your child, your family, your friends and their families, and the community.

The Tdap vaccine is available for adults at Florida Department of Health in Okaloosa County for $60.23.
