Copies/Reprints

There is $15 charge for records transcription and issuing the certified DH 680 form for each request.

If you are moving from another state to Okaloosa County, we can transfer your immunization information to FL Shots by using your current immunization record from any state.

This record will be transferred into FL Shots and a new certified Florida Immunization DH 680 record will be issued. No appointments are necessary to drop records off; however, depending on clinic volume transcription may take up to five business days to be completed.

For DH680 requests, parents may walk-in and complete a request form or download FL DH680 Request Form (PDF 208KB) and bring to the health department during immunization operating hours. A unique PIN# (per child) will be generated so DH680 can be electronically retrieved by the parent/guardian.

Immunization records request using signed release of medical information may be faxed to 850-833-3442 or emailed to OKImmunization@FLhealth.gov

Immunizations are provided free for children, ages birth through 18 years. Before a child may enter childcare or attend a school, proof of immunizations or an exemption must be provided. 

The following vaccines for children are available at the Florida Department of Health in Okaloosa County:

  • DTap: Diptheria, tetanus, and pertussis
  • HepA: Hepatitis A
  • HepB: Hepatitis B
  • Hib: Haemophilius influenzae B
  • HPV: Human papillomavirus
  • Influenza                      
  • MMR: Measles, mumps, and rubella
  • MCV4: Meningoccal (Menatra)
  • PCV: Pneumococcal conjugate
  • Polio                
  • Rotavirus                     
  • Tdap: Tetanus, diptheria, and pertussus
  • Varicella: Chickenpox

The Tdap vaccine is available for adults at Florida Department of Health in Okaloosa County for a fee of $61.23.

For a religious exemption (DH 681 form) request, parents can schedule an appointment by calling 850-833-9246.

DOH-Okaloosa does not provide travel-related immunizations.
