If you need a copy of immunization records from DOH-Okaloosa, no appointment is required; however, depending on clinic volume we may not be able to provide the documents immediately and the parent may need to return to pick it up.  If you are coming to the facility please call the number posted outside before entering the building. 

If you are moving from another state to Okaloosa County, you can transfer your immunization information to FL Shots by presenting your current immunization record from your previous state.

The record will be transferred into FL Shots and you will be issued a new Florida immunization record. No appointments are necessary, but please allow three business days for your form to be completed. If you are transferring a child into the Okaloosa County school district and need information on school physicals and a list of providers, please see School Health.

Immunizations are provided free for children, ages birth through 18 years, through the Vaccines for Children Program (VFC). Before a child may enter childcare or attend a school, proof of vaccines must be provided. Immunizations can protect your child, your family, your friends and their families, and the community.

The Tdap vaccine is available for adults at Florida Department of Health in Okaloosa County for $55.53.
